OK,

Here is a new one that specifically mentions New Guinea and Melanesia in the menu description and also uses the Covarrubias Tiki image. This is off the menu from the Polynesian Village Restaurant in the Edgewater Beach Hotel in Chicago. This place could date into the early 1950s.
