That thing's definitely from Bali, but not technically "balinese" style. They're all over the place around here for real cheap in all the crappy swapmeet bali import carving stands. They also do them with the inlays in sorta African styles too.

The painted patterns look a little PNGish, but to call it a PNG mask, or a tiki mask, would be a real stretch. I think you could get away with calling it a "Tribal Mask."
