Doesn't count as a "location," but here's a Don the Beachcomber menu for Continental Airlines. Interesting in that the cocktails listed don't include Don mainstays like the Zombie or Navy Grog, but the Proud Bird (for which I've never seen a recipe), Chi Chi and Mai Tai. . "Proud Bird" was the name of Continental's 747 to Hawaii.
